Please follow these four steps to open a new file with Medisys:

1. Execute our Fee Schedule Contract (Click). This contract will answer questions you may have regarding our fee structure, travel, testimony, etc. Please indicate requested services, sign, and date.
2. Complete the Patient information form (Click), indicating important details about the case.
3. Send all relevant medical, education, and vocational records to us in electronic (PDF) form. Please include an index of what is being sent. Records can be placed on a disc/drive, or in a secure online file-sharing program.
4. Please send the completed fee schedule contract, patient information form, and appropriate retainer (see Fee Contract) to our mailing address. MediSys does not accept credit cards.
